Your membership includes four in-office visits per year, comprehensive lab work drawn in office, personalized treatment planning, direct access to your provider via text or email, and prescription management. Medications and optional specialty tests are not included in the annual fee.
Yes, your initial comprehensive labs are included. Additional labs or specialty screenings (like genetic testing or Galleri® cancer screening) may have separate costs.
Yes, all new patient paperwork must be completed through the patient portal before your first visit. This allows your provider to review your history and create a personalized care plan. Incomplete paperwork may result in a rescheduled appointment.

Before beginning Bioidentical Hormone Replacement Therapy, a genetic screening is required to assess your personal risk factors. This one-time test is $250 and not included in your membership fee.
